Milking the prostate

Prostate milking stimulates the prostate gland, also known as prostate massage.  It can be performed for either medical or sexual reasons.  Correct me if I am wrong, but for some men, massaging your prostate gland can feel taboo, dirty, or wrong.  So often, we focus on the sexuality of the act rather than the health benefits associated with it.  It doesn’t help when the way it is depicted socially is of a doctor’s office, bending over, and reeks of discomfort and embarrassment.

Prostate massage is gaining more acceptance as the health benefits associated with the practice gain traction.  Sexual health is a significant contributor to our overall health and well-being.  So it makes sense that something that can be for sexual reasons also benefits our overall health.

What are the benefits?

The benefits of prostate massage fall into two categories: health and pleasure. 

For starters, let’s recap what the prostate gland is.  The prostate is part of the male reproductive system.  The male reproductive system includes the penis, prostate, seminal vesicles, and testicles.  It is a walnut-sized gland located below the bladder but in front of the rectum.  The location is why you can manually massage it from the rectal passage.  The prostate gland surrounds the urethra, which is why we get issues with bladder control and flow when it becomes enlarged.  Finally, the prostate gland produces fluid that mixes with sperm and seminal vesicle fluid to become what is known as semen.

Prostatic massage helps to clear the prostatic ducts of excess seminal fluid.  The duct runs between your prostate and the rest of your reproductive and urinary system.  The act of massage helps release fluid which can assist with the pain and discomfort associated with inflammation and swelling commonly associated with Benign Prostatic Hyperplasia (BPH) and Prostatitis.

What does the evidence say?

Most studies on prostate massage’s health benefits have been small and relied on participants’ self-assessments.  It is for that reason most doctors are hesitant to recommend the practice.  However, research has shown that some men may find symptomatic relief from prostate massage.  For example, one study followed 200 men over two years who suffered ailments such as enlarged prostate, overactive bladder, or chronic pelvic pain syndrome.  The study tested the benefits of an in-home use prostate massage device in treating these conditions and found that most men experienced an improvement in their lower urinary tract symptoms.

Prostate massage is a therapeutic support device used with other treatment options based on consultation with a health care professional.

Indeed there are some risks associated with this?

The tissues lining the prostate and the rectum are delicate and vulnerable to cuts, tears, and abrasions.  Massage performed rough can damage these tissues, resulting in soreness or increased risk of infection.  In addition, overly aggressive prostate massage can aggravate hemorrhoids.

While some studies have shown the therapeutic benefits of massage with antibiotic therapy for Prostatitis, it is discouraged for acute bacterial Prostatitis.  The massage is said to further increase inflammation and could inadvertently increase the spread of bacteria.  If you have Prostatitis, talk with your healthcare professional on whether prostate massage is advised for your unique situation. 

Why are more people talking about prostate massage nowadays?

Well, the increased interest in prostate massage is due to the pleasure that can come with it.  As the massage can help clear the ducts of excess seminal fluids, some men find improvement in their overall sexual function as a result.  In addition, massage promotes blood flow, so the increased blood flow to the area can sometimes lead to improved erections. 

Now, I don’t have a prostate, so I can’t really tell you what a prostate orgasm feels like.  Your best option is to ask some male friends if they have ever experienced one and see what they say it feels like.  What has been said about prostate orgasms is that it is different from genital orgasms in that it feels like it comes from inside the body and is often uncontrollable.  However, as we all know, sex has different strokes for different folks.

Okay, so I’ve weighed the risks and want to give it a shot.  How do I do it?

If you have problems with your prostate, my advice will always go see your healthcare professional and ask about prostate massage.  They will be able to direct you to services or a qualified healthcare professional that can perform the massage for you.  I believe if you have an already enlarged prostate or are experiencing problems, getting a qualified prostate massage first will always be the best solution so you can understand how it feels and how it is done. 

Having said that, there are ways to massage at home.  One is through manual massage, and the second is through a prostatic massager.  I recently attended Sexpo and spent some time talking to conveyors about introducing prostate massage into the bedroom and what sort of massagers are suitable for beginners.  I knew you would have that question for me.  

Side note: your adult store proprietors are a great source of knowledge regarding the devices on the market and what works well in certain situations.  I highly recommend moving past any source of embarrassment or discomfort and opening a dialogue with these people in the store.

What was recommended?

Nearly every conveyor at Sexpo recommended an anal balloon pump was a great beginner device for anyone interested in prostate massage.  This recommendation was based on the following reasons:

  • Usually smaller to insert as they are deflated so they can feel less invasive
  • Control over the sizing by inflation
  • Quick deflation allows for more comfortable insertion and removal
  • Safety in a release mechanism.  The device can be deflated quickly before removal

There are other great non-inflatable prostate massagers out on the market.  However, if you are a beginner to prostate massage, the inflatable was considered safer, easier, less daunting, and allowed more control over the experience for the user.

Okay, I bought one.  Now what?

Well, if we are massaging the prostate through the rectum, we will need everything to be as clean as possible before you start.  Some key things to always have on hand include:

  • Your massager
  • Lube
  • Sex toy cleaner
  • A towel
  • Gloves, or practice good hand washing techniques
  • Personal preference on whether you want an anal douche or enema kit beforehand

Getting your partner involved is always a good idea as then can help control the insertion while you learn to adjust and direct them.

As a rule of thumb, here is a general step-by-step guide:

  1. If you are using an enema kit or anal douche, follow the steps for the kit
  2. Place your towel in whatever area you have decided to conduct the massage in
  3. If using gloves, put them on, or practice good hand washing techniques
  4. Use a generous amount of lube on your anus and your finger
  5. Insert your finger into your anus slowly, pausing as you breathe and relax and allow the muscles of your anal cavity to adjust and relax
  6. Carefully and gently feel around till you locate your prostate gland.  The prostate feels like a rounded lump roughly 4 inches inside the rectum towards the root of the penis.  If you cannot find it or cannot reach it, do not worry
  7. Depending on the size of your massager, you may want to insert an additional finger to allow your anus to expand and relax.
  8. Generously lubricate your massager
  9. Get in a comfortable position for yourself
  10. Slowly and gently guide the massager into your anus to a point you are comfortable with
  11. Feel for the massager to come into contact with your prostate.  That feeling should feel similar to when your finger touched it
  12. Gently move the massager into a placement you feel comfortable with, and from here, you can experiment slowly with different levels of inflation
  13. Engage any of the additional features of the massager as desired.
  14. Deflate the massager before removing
  15. Slowly and gently remove the massager and clean it properly.

What’s the take-home message here?

Deciding to incorporate the practice of prostate massage will come down to personal preference, and so will the prostate massage device you choose to use.  Please be mindful of the risks associated with the practice and always consult your health care professional if you have any concerns or questions regarding your prostate health.
